That looks a bit different to the version I have, but if you click on any of the blank buttons on the window on the left, it should bring up a keyboard on screen (and a representation of the mouse). You can then click on any key (or combination) to assign that key to the button. Once you've set it all up, save using the disk icon on the bit on the right where it says "untitled profile".
I'm trying this for over an hour and its not working,is there anyone with already working profile for MGS2 /xbox360 controller because this is driving me nuts :'(

a better solution would probably be to use the XBCD drivers to make your 360 pad Direct Input instead of XInput, works a treat for most older games

if you use XBCD you won't need xpadder, for xpadder you are trying to emulate keyboard and mouse using the pad, with XBCD you are changing the pad to Direct Input so as long as the game has controller support (looking at your screenshot I'm assuming it does) you are good to go
it comes with a utility to change which axis the sticks use and also change the triggers to buttons instead of analogue triggers.

install the drivers then go into device manager, right click on 'Xbox 360 controller for windows' and click update driver software, then 'let me pick from a list of device drivers on my computer' and the xbcd driver should be listed so chose that, if it isn't you'll have to go back and browse for the driver
once your pad is recognised as XBCD Xbox 360 Gamepad the game should recognise it and let you configure controls with it (it should recognise the right stick as well which is normally the problem with using a 360 pad for older games) if it doesn't recognise everything you need to use the setup utility which allows you to change what every button, stick and trigger does and how it's recognised

it's a bit of a pain but well worth it to use the 360 pad properly with older games
