Regarding missions, that's why I put in the question mark. And surely, the point of mods is to go beyond the intended design of the game? I for one would welcome the chance to take three squad members.

I have HP sauce with everything, including roasts.
When I was young I never found anything that ketchup made worse. My parents wouldn't let me try it with ice cream though. Tyrants.
ME1 needs fewer weapons, but more variety in those it has. I'm playing it atm, and the only weapons I can tell apart are the normal assault rifles (which all sound the same and have the same rate-of-fire), and the Geth Pulse Rifle (which pew-pews the crap out of everything). Minor variations in accuracy and cooling do not help.
Mass Effect 1? A shotgun with explosive rounds and lots of coolant. You shoot people and they fly off onto the sky. Especially insane paired with singularity.
Well mass effect 3 brought back explosive shells, sure it has a time delay but since I got this pistol I've been reducing all of my enemies to tasty meat... Except the metal ones. Now I just need a badass expanded clip and a scope and I know which gun I'm using until the end of the story.

Not sure you can call it a "mod", but I played ME2 with something (what it was, I can't remember now) that made that dreadful scanning unnecessary.
ME needs inventory and interface mods so badly. Just making weapons of the same type stack would be such a godsend.
It boggles my mind why they didn't at least sort the equipment alphabetically instead of the "by time acquired, and something else" rule.

Also a "you already have three of these, so I will auto-sell the next one" feature would have made me very happy indeed.
Speaking of which, isn't it about time that these RPGs that give you a big pack of followers but only allow you to take a couple on a mission develop a good way of handling their inventories? In my headquarters, I want to select the equipment for each follower from the list of all my items not currently equipped by me (so, allowing me to equip the same object on more than one person). If I select followers for my next mission and there is not enough of a certain equipment type to go round, then I have to decide who gets it and what the other person gets.
This would make choosing a different follower combo on each mission so much more convenient.
