I tend to jump around in games too much to put a huge amount in to any one game these days.
My most played on Steam is Skyrim with only 60 hours. NS2 is 2nd despite me only getting it last month, at 40. I have tonnes of games somewhere from 20-30, I tend to get bored at this point.
That obviously excludes MMOs, WoW was at about 250 days before I stopped playing...So many regrets. Battlefield 3 and Starcraft 2 probably top Skyrim as well.

Mount and Blade Warband 189 Hours
Tf Classic 177 Hours
Coh ToV 139 Hours
Oblivion 80+ Hours
Then it starts to drop sharply.
Noteworthy : Ns 2 will be entering the top 5 in a Month at this Rate.
Scary : I havent played the lower 1/3 of my 220+ Games a *single* Minute.

241 hours on Mount & Blade: Warband. Keep in mind I generally avoid using Steam whenever possible and go out of my way to find a DRM free physical copy or use GoG, so there might be another game out there that beats this time. The second on the list is of course Skyrim with 170 hours. Are the results unreliable if I used Steam's offline mode?

I have no idea of the time but with sixteen playthroughs of Deus Ex it's probably my most played game. Morrowind is behind it with some 200 hours for sure.
Then according to steam I have:
150hs on TF2
92hs on Sword of the Stars (which should be higher as I started with a non steam vanilla version when it was released)
69hs on Mount & Blade Warband

Well on Steam my top list is:
Mount & Blade (vanilla): 281 h
Dragon Age: Origins: 270 h (wife played through this twice so there's a lot of that)
Killing Floor: 244 h
Skyrim: 180 h
Modern Warfare 2 multiplayer: 133 h
After that it drops below hundred so won't bother listing.

The Last Remnant: 166 hrs
Fallout New Vegas: 148 hrs
Damn, I spent a lot of time spawning the rare monsters in TLR.

My Steam statistics are mostly pointless because some of the games don't count my play time for some reason (apparently I played 58 minutes of New Vegas) and also because I spent nearly two years with Steam almost constantly in offline mode, but here we go:
Football Manager 2011: 285 Hours
Duels of the Planeswalkers 2012: 251
Blood Bowl: 130
Football Manager 2012: 113
I would guess at around 300 hours of Oblivion and maybe 200 hours for Morrowind, probably at least 500 more hours of various Football Managers (I remember my 2005 game lasted till 2029 which must have taken a while), and goodness knows how much Baldur's Gate 2. Also lots of Paradox games and Civ 2 will be very high on the list, but it's really hard to estimate how long I play them for because they make time disappear.
My father has reached 1204 hours of Civilization 5 and also has 327 hours of Football Manager 2009.

Sorry. Steam started to count around 2008/2009. Before March 2009 but after August 2008 (cause it counted my empire TW gameplay but it did not count my Uplink which according to my steam account history I purchased on 17 Aug 2008)
It will not record the time if you are in offline. Otherwise I would have another 10 hours in ME2 and 30 in ME1. The time recording happens after you close the game. If you skip that part and close steam before game, it will not record time. At least that's my understanding of how it works and why there might be discrepancies.
but yeah. offline = no time keeping.

According to Steam:
Skyrim: 208 hours
Space Pirates and Zombies: 106 hours
Far Cry 2: 78 hours
Company of Heroes: 78 hours
Fallout 3: 69 hours
Probably fairly accurate for the first two, but not for the rest; it's rather a lot more, especially for the last two.
Minecraft is likely the game I've played the most in the ~30 years I've been gaming, but BF1942 must be up there too.
And Oblivion and Dune 2 and Starcraft and X-Com and Fallout 1 & 2 and ...
